If you have recently had a bad experience with an [ADF] member, please fill out this form. Please note that we aren't likely to act on complaints that do not carry any supporting evidence (screenshots or video).

Member Name		The name of the member you wish to lodge a complaint against.
Time and Date of Incident		When did you have a bad experience with one of our members?
Location of Offence		Please explain where one of our members was misbehaving (e.g. the name of a BF3 server)
Description of Offence		A brief description of what the [ADF] member was doing.
